北部中部の大都市では、昔と違って今はかなり開いています。確かベルルスコーニ時代に制度改正して日曜でも自由に商売できるようにしたという記憶があります。
南部の小都市などではまだ事情が違いますが。

ミラノの商業施設の営業時間については下記に詳しく記載があります。

多くの店が日曜は休みですが大聖堂周辺の大型店やデパートは日曜日も休憩時間なしで営業しています。
The normal opening times for Milan (subject to change):
Mondays: 09:30 pm - 07.30 pm, some little shops are closed on Monday mornings
Tuesdays-Saturdays: 09:30 am - 07:30 pm
Some little shops have a lunch break from 12:30 pm -03:30 pm
The majority of shops still close on Sunday, but the shops in the center of Milan around the cathedral square are continuously open and more and more retail shops and department stores such as La Rinascente and Coin are open on Sundays.
Large stores such as La Rinascente or Coin are also open on Sundays with continuously hours. The opening hours are from 10 a.m. to 8 p.m.
